Choose a contact form for the message you need
A general question usually needs fewer fields than a project inquiry. Choose a layout that gives visitors enough room to explain their request without asking for unrelated details.
Contact Form V20
Choose it for: a compact contact us form with a subject line.
Contact Form V20 places name and email together, followed by subject and message fields. The centered card is a straightforward starting point for a general contact section.
Contact Form V19
Choose it for: a project inquiry that needs an extra selection field.
Contact Form V19 uses a wider layout with name, email, a dropdown and a message area. Adapt the dropdown options to the information your team needs to route or assess an inquiry.
Contact Form v3
Choose it for: one interface with separate contact and quote modes.
Contact Form v3 switches between Say Hi and Get a Quote. The quote choice reveals additional dropdowns, while its background image and green form panel give it a more prominent visual treatment.
Build a complete contact us page around the form
Tell visitors what the form is for and what information to include. Add the contact details relevant to your business, such as a support address, opening hours or location. Only promise a response time that your team can meet. Keep a general message form short; introduce extra fields when the type of request makes them useful.
Configure delivery before going live
A form layout is only one part of receiving messages. Contact Form V19 and V20 include JavaScript that posts to a PHP mail-handler path; configure and test the handler on your hosting, or connect a form service. Contact Form v3 needs its submission destination connected. Verify delivery to the intended inbox and check what visitors see when a request fails. Browser-side validation alone does not confirm that a message was delivered.
Check dependencies and match the form to its purpose
All three reviewed demos use standalone CSS and JavaScript rather than a Bootstrap stylesheet. Test the selected download within your own page for style conflicts, keyboard use and mobile layout. A contact form collects an inquiry; use registration form templates for account creation and login form templates for returning users.
