With this stylish free dark background contact form template, you easily go against the grain and make a unique appearance. The template is fully compatible with all modern devices, smartphones, tablets and desktops.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 253 KB (it was 275 KB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. The form sends through the included PHP mail script without leaving the page; set your own address at the top of the file in the php folder. Validation uses the browser’s built-in checks, shown in the design’s own style.
