The best free minimalist contact form template with a round CTA button and a budget picker. Of course, you can additionally modify the default settings, tailoring this free snippet to your liking.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 191 KB (it was 1.3 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. The form sends through the included PHP mail script without leaving the page; set your own address at the top of the file in the php folder. Validation uses the browser’s built-in checks, shown in the design’s own style.
