Enrich your professional website with this free business contact form template that works great for agencies. It comes with additional fields and text on the side that you can use to convince the potential client further.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 76 KB (it was 1.3 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. The form sends through the included PHP mail script without leaving the page; set your own address at the top of the file in the php folder. Validation uses the browser’s built-in checks, shown in the design’s own style.
