Sometimes, you need to showcase the exact location of your business, and this free contact form with Google Maps template is a perfect solution to make it happen. Along with that, you can also add all the other necessary information at the bottom of the template.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 257 KB (it was 277 KB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. The map is a keyless Google Maps embed, so there is no API key to set up and no “for development purposes only” warning. The form sends through the included PHP mail script without leaving the page; set your own address at the top of the file in the php folder.
