If you search for a predefined template, this free general inquiry contact form snippet will do the trick. Give your site visitors an option to reach out to you at any time they want, without picking up the telephone.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 77 KB (it was 2.3 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. The form sends through the included PHP mail script without leaving the page; set your own address at the top of the file in the php folder. Validation uses the browser’s built-in checks, shown in the design’s own style.
