Even though this free food class registration form template is for culinary intentions by default, you can use it for something else, too. It features a split-screen design, with a date picker, drop-downs and fluid layout.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 881 KB (it was 1.1 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. Date fields use the browser’s own date picker, so phones get their native calendar. The drop-downs are real select menus, styled as before, so their values are sent with the form.
