An enrolment form for a made-up sixth form college, with a navy and gold header, a side panel of key dates and three numbered sections. The date of birth works out the student's age on the first day of term and makes the parent or guardian section required under 18, or collapses it for older students. The course picker groups A levels, vocational and foundation courses.

It is plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins, and the download is 472 KB. Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same.
