Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 87 KB (it was 1.9 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. The range slider is a few lines of plain JavaScript, with mouse, touch and keyboard support. The drop-downs are real select menus, styled as before, so their values are sent with the form.
