With this free customer registration form template, everyone can create an account quickly and conveniently. The widget features name, DOB, gender, phone number and password fields with additional (optional) sections.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 299 KB (it was 2.1 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. Date fields use the browser’s own date picker, so phones get their native calendar. The drop-downs are real select menus, styled as before, so their values are sent with the form.
