Description

This free modern registration template includes multiple predefined fields for name, gender, DOB, email and more. The layout is fully responsive, adjusting to mobile and desktop instantly, with no framework to load.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 260 KB (it was 1.0 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. Date fields use the browser’s own date picker, so phones get their native calendar. The drop-downs are real select menus, styled as before, so their values are sent with the form.
