A phone-number sign-up for a made-up same-day courier app, designed for thumbs first and shown as a tall card on a yellow page on desktop. Step one pairs a country-code select with the national number and checks its length for the chosen country; step two takes the 6-digit code in one large field with one-time-code autofill. The main button sticks to the bottom of the screen on phones, and a resend button counts down from 30 seconds.

It is plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins, and the download is 77 KB. Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same.
