A convenient free subscription form template with a split-screen design and a responsive layout. The widget features name, email and password fields with a checkbox for terms of service.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 318 KB (it was 1.5 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. Validation uses the browser’s built-in checks, shown in the design’s own style.
