A full-blown sign up form with no hidden fees, readily available to put it into play. With the split design, you can also include an image with a gradient overlay on one side, spicing things up. Keep in mind, the image part disappears on mobile.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 326 KB (it was 2.9 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. Validation is a few lines of plain JavaScript that show the design’s own error messages. The floating labels need no library either.
