Description
A warm volunteer sign-up form for a made-up community food pantry. After contact details and an age group, volunteers pick the roles they would like as checkbox cards and tick free slots in a weekly grid of days by morning and afternoon. Quick-pick buttons fill weekday mornings or weekends, and a tally shows the number of slots and hours offered.
It is plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins, and the download is 188 KB. Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same.
- Availability grid is a real table with row and column headers
- Every slot checkbox is named, e.g. Monday morning
- Quick picks for weekday mornings, weekends and clear all
- Live tally of slots and weekly hours
- Role cards need at least one choice, the grid needs at least one slot
- Grid and side panel stack cleanly on phones
