If you’d like to mix a minimalist look with a touch of creativity, then this free trendy contact form template is ideal for you. Besides the contact fields, there’s also space to add extra company details, even a telephone number and email address.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 110 KB (it was 1.2 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. The form sends through the included PHP mail script without leaving the page; set your own address at the top of the file in the php folder. Validation uses the browser’s built-in checks, shown in the design’s own style.
