Description
A post-delivery satisfaction survey for a coffee roaster. It asks for a 1 to 5 star rating built from a native radio group, a 0 to 10 likelihood-to-recommend scale and optional comments with a character counter. Submitting shows a thank-you state with a discount code in place of the form.
It is plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins, and the download is 284 KB. Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same.
- Star rating is a real radio group, so arrow keys work
- Hover preview and a word label for the chosen rating
- 0-10 NPS scale that wraps to two rows on phones
- Errors appear only after interaction and are announced politely
- Character counter on the comments field
- Success state takes focus, with an Edit my answers button
