Description
An end-of-term course evaluation for a made-up college history course, with a navy and gold academic header. Part A has four styled range sliders for overall rating, weekly hours, pace and feedback quality, each with a live value badge and spoken aria-valuetext. Part B has open questions, one required with a minimum length, and the success state recaps the slider answers.
It is plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins, and the download is 267 KB. Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same.
