A simple and easy-to-use free travel booking form template with date options and a number of guests selection. The layout is 100% mobile-friendly and light, with no framework to load.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 250 KB (it was 256 KB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same.
