A bold diner-style application for a made-up wood-fired restaurant, with a dining-room photo header and a hard red offset shadow. Applicants choose a position, pick shifts from ticket-style checkboxes, choose weekly hours on a segmented control, set a start date and answer a right-to-work question. Answering No shows an honest note without blocking the application, and the phone number formats itself as it is typed.
It is plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins, and the download is 550 KB. Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same.
