Description

Grab this modern and mobile-ready free split-screen contact form template and integrate it into your website. One side features the form and the other company details. On mobile, it changes from horizontal to vertical positioning.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 257 KB (it was 273 KB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. The form sends through the included PHP mail script without leaving the page; set your own address at the top of the file in the php folder. Validation uses the browser’s built-in checks, shown in the design’s own style.
