Description

With our vast collection of free contact form templates, we make sure there’s something for everyone. This one includes a Google Maps background and a form with a top banner image. You can also add a title and some additional text.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 216 KB (it was 2.6 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. The map is a keyless Google Maps embed, so there is no API key to set up and no “for development purposes only” warning. Validation is a few lines of plain JavaScript that show the design’s own error messages.
