Description

With the split-screen design, Contact Form v13 is way more than your average free template. One part comes with all the necessary sections for users to get in touch with you. And the other part comes with integrated Google Maps.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 202 KB (it was 2.8 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. The map is a keyless Google Maps embed, so there is no API key to set up and no “for development purposes only” warning. Validation is a few lines of plain JavaScript that show the design’s own error messages.
