A dark restaurant reservation form with a candle-lit photo panel beside it. Guests set the party size with a stepper, pick a date and then choose from dinner time slots between 17:30 and 22:00, where some slots show as full for each date. Seating is chosen from icon cards, with an occasion menu, special requests with a character counter and contact details. A confirmed booking is shown as a reservation ticket with a reference number.

It is plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins, and the download is 442 KB. Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same.
