Description
If you are searching for a more basic contact form that pops over a Google Maps background, opt for Contact Form v20. It’s very easy on the eye and guarantees an excellent experience on different screen sizes and devices.
Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 69 KB (it was 2.5 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. The map is a keyless Google Maps embed, so there is no API key to set up and no “for development purposes only” warning. Validation is a few lines of plain JavaScript that show the design’s own error messages.
