Description
Instead of offering a subject field to fill out manually on a contact form, you can include a drop-down for quick category/topic selection. Contact Form v18 delivers just that.
Also, you get social media buttons and an image background that covers more than half of the screen.
Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 454 KB (it was 4.7 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. The drop-downs are real select menus, styled as before, so their values are sent with the form. Validation is a few lines of plain JavaScript that show the design’s own error messages.
