Description

A modern a simplistic free contact form template with validation and autocomplete. You can also add additional contact details at the bottom of the widget and use Google Maps as a background (with a gradient overlay).

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 212 KB (it was 3.3 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. The map is a keyless Google Maps embed, so there is no API key to set up and no “for development purposes only” warning. Validation is a few lines of plain JavaScript that show the design’s own error messages.
