Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 450 KB (it was 2.6 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. Validation is a few lines of plain JavaScript that show the design’s own error messages.
