Description

A simple, versatile and easy-to-use contact form template that comes with no hidden fees. It uses CSS3 and HTML5 with a user-friendly code structure for users of all skill levels.

Updated September 2026: this snippet is now plain HTML and CSS plus a few lines of vanilla JavaScript: no jQuery, no Bootstrap files, no plugins. The download is 341 KB (it was 3.1 MB). Use it on its own, or drop it into a Bootstrap 4, 5 or 6 project and it looks the same. The hover tilt effect is plain JavaScript. Validation is a few lines of plain JavaScript that show the design’s own error messages.
